Kaiser Permanente: and others are not ‘insurers’ and as such can deny treatments and medications which do not fall under their guidelines: most recently there was a supreme court ruling which stated essentially that ‘kaiser and other similar plans could not be sued even if they denied treatment which was useful’
Insurance companies on the other hand can deny treatment but can be sued if they do not provide community standard of care.

Although no plan will pay for all the costs associated with your medical care, some plans will cover more than others.
